This project page will be the primary page for all Live-Well surveys and evaluations in the 2025-26 financial year.

This page is in 'protected' mode found on the 'Visibility tab in the project page 'settings'; so that it is unable to be searched or unintentionally found through a Google search or on the HYS home page.

Please ensure that only the 'stand-alone' link found on the 'Share' tab in the edit survey function is shared with your target stakeholder users.

NB Both the project page and the survey needs to be in 'Published' status to use this link. Please talk to a site administrator to change the project page status.
